Patients within these areas can now receive care from a multi-disciplinary team for more effective management of diabetes and related conditions, through access to the ancillary and support services provided by Diabetes Singapore. For instance, patients can easily access nurse counsellors for individualised advice on how to manage their conditions, including lifestyle and dietary modification. Diabetes Singapore works closely with the PCN care coordinators and healthcare professionals to provide a seamless and one-stop patient experience.
